Drelzna von Zarovich: An ancient warrior-vampire of the von Zarovich line, older than even Strahd, who for reasons unknown was interred in the Sanctum Maleficum for thousands of years by her mother Iggwilv. For the last seven years she's waged war with the troll king Rend for dominion of her mother's dungeon, though her true interest is the Planar Nexus. There is a sense of loneliness about her, as everything and everyone she ever knew is now dust and ruins. Despite her adviser's disapproval, she accepted the three vampires' offer to deal with the troll king and the unknown mage behind the throne.
Fraz-Urb'luu: A filthy, wretched creature hunched and ragged of gait, with the fly-ridden head of a goat and hooves, hidden under a soiled indigo robe. He hugs a massive tome with dozens of latches and acts as Drelzna's adviser. He has the ability to vanish and reappear in the blink of an eye, and Drelzna mentioned he was a powerful mage besides. He despises the group, convinced they will fail at their task and that alliance with them is a waste of time.
King Rend (mentioned): King of the Tsojcanthi trolls, his power is legitimized firstly by his unusual size and strength even among his own kind and secondly a symbolic iron crown that has belonged to the line of troll kings since they settled the caverns. Drelzna believes he is just a brutish figurehead, and that some powerful mage truly lords Tsojcanth City.
Iggwilv (mentioned): The Witch-Queen of Rakados, once the ruler of the ancient city-state of Izolda, she was a powerful archmage and demon conjurer in the days before the Gloriandeline and Nors empires. So powerful, she was able to summon demon lords and princes. She is the mother of Drelzna von Zarovich, whom she interred in her caverns of power Tsojcanth for reasons unknown. By the way in which Drelzna regards her, they were quite close.

Could the fist-sized gem of swirling blue, white and green embedded in Drelzna's sarcophagus be another Heart?
The Crown of the Troll King
The iron crown worn by King Rend is the symbol of power to the Tsojcanthi trolls. Drelzna desires to crush it in her hands while the larger troll populace watches, forever making herself their queen.
Mage in the Shadows
Drelzna suspects King Rend largely a figurehead, for her adviser Urb'luu's scryings and all her secret agents are quickly dispelled or dispatched. He or she must be a very powerful mage, who will also need to be found and dealt with for Drelzna to truly rule the caverns.
